Mayo Clinic Hospital in Arizona (three days after the collision) complaining of frontal headaches and pain throughout her body (itemized in the medical records as right chest pain, right upper quadrant pain, right ribcage pain, lower abdominal pain, right flank pain, bilateral calf pain, bilateral shin pain, bilateral leg pain, pain radiating from flank/lower back to lower right leg). -

Consulted with Philip Lichtenfeld M.D., at Beverly Hills Doctors Medical Association. An X-ray of Ms. GARCIA lumbar spine was ordered, which revealed moderate disc space narrowing and facet hypertrophy at the L3-L4 and L5-S1 levels. -

A CT scan of the brain was ordered. Healthcare providers diagnosed Ms. GARCIA with intractable headaches and prescribed Norco and Zofran. -

The medical records document her as suffering from pain/spasms, upper back pain, mid back pain, lower back pain, bilateral shoulder pain/spasms/aches, left scapular pain, bilateral arm pain, bilateral hand pain/weakness, left hand numbness, abdominal pain, left lower quad pain, groin pain, gluteal pain, bilateral leg pain/weakness/spasms, bilateral thigh pain, bilateral feet burning sensations, nausea, dizziness, vomiting and difficulty with balance and Dr. Pazmino ordered several studies. -

Dr. Vassilev diagnoses her with cervical region spondylosis, mid cervical region disc displacement, lumbar region intervertebral disc degeneration, low back pain, lumbar region radiculopathy and left side sciatica. He advised Ms. GARCIA that she was a candidate for cervical facet injections. -
