  • Space Shuttle Columbia Disaster

    The Space Shuttle Columbia disaster occurred when the Space Shuttle Columbia disintegrated during re-entry into Earth's atmosphere. This was caused by damage to the shuttle's thermal protection system, which occurred when a piece of foam insulation broke off from the external fuel tank during launch and struck the left wing of the shuttle.
