    Started from Hillfarm Dairy in Melrose Park, Ilinois. It caused a confirmed number of 16,284 ill possibly 200,000 ill. There is a confirmed death of 2 people, but there might have been more as 12 deaths. Contamination came from contaminated Milk.

    E. Coli from Jack in the Box
    four children died and about 700 others ill around the Seattle area, and part of Pacific Northwest. Source of contamination comes from undercooked hamburgers.
    Reports started around November 1992, but wasn't confirmed till January 12, 1993.
